The information gathered is very helpful to your child’s school as it works together with The Winchendon Project to develop and improve student support services. What is this survey about? This survey is designed to help gather information about your knowledge and experiences related to behavioral health (including substance abuse and mental health) and your son/daughter. The information you provide will help us better support the emotional, social, and physical health of all students.
Is it confidential? Your response is completely confidential and anonymous. Surveys will not be processed at the school. Surveys will be analyzed by an Evaluation Consultant who will develop a report based on all surveys received. You may skip any question if you do not feel comfortable answering it.
